the slope intercept form is the rate of change between any two points on a line it always is the same. The constant rate of change is called the slope of the line. Slope is the ratio of the rise, or vertical change, to the run, or horizontal change.

So Slope intercept form is basically Rise over Run.

Slope-intercept form is the formula:
Y=mx + b
The m= the slope
The b= the y-intercept
For example,
If you had the slope intercept form
Y=5x + 6
5= slope
6= y-intercept
