Failure to document information properly result in POOR OUTCOMES WHICH CAN HAVE SEVERE CONSEQUENCES.
Information documentation is very important in all spheres of life, it has a lot of advantages, but the most prominent of them is that, it allows other parties to know the things that has been done, the way it has been done and what remains to be done, in case the original handler of the document is no longer available. Not documenting information properly can lead to a lot of negative and serious consequences. For instance, not document a patient information properly in the hospital can lead to the unnecessary death of that patient.
