My actual answer to this question is the show "Naked and Afraid" because contestants suffer from all types of things on the show from bug bites to sprang ankles all to survive 21 days, naked, in the wilderness. Then my own personal answer wants to be "The Purge" even though that is not a reality show in the sense that you are asking.
