The new plan needs to be reviewed by the patient, case manager, doctors, any specialist, and the patient's insurance company. Any medication changes, lab orders, and the plan itself needs to be written clearly and exactly what everyone agreed on. Any new appointments should be scheduled before the patient leaves.
(My personal opinion as I have had my care plan changed several times.)
