The registered nurse, or RN, should delegate the unlicensed assistive personnel, or UAP, to keep the clients skin nice and dry, to use a pressure relieving mattress and to turn the client every two hours.

the licensed nurse should assign the UAP a duty of keeping the skin of the patients dry. skin evaluation of the skin is the responsibility of the nurse although the unlicensed assistance may be directed to report any redness noticed. documentation of results in Braden Scale is the work of the RN.
