E/M means evaluation and management coding. E/M is a major part of the medical coding process which supports medical charges. E/M standards and guidelines are used by private health insurance organizations as the standard guidelines for ascertaining the extremity and type of patient ailments. The elements of the e/m level that are subjective include; Chief Complaint (CC), Review of System (ROS), History of Present Illness (HPI), and Past, Family, and Social History (PFSH).
