If there’s a problem with the heart this usually means it can’t do it’s job efficiently. most of the time, having a heart problem affects breathing as there’s not enough oxygen being pumped around to the lungs or the brain (or anywhere in the body)
An example sentence: heart disease can cause breathlessness if your heart is unable to pump enough blood to supply oxygen to your body.

Because with problems with your heart which makes blood pump out slower which mean there is less blood getting to your brain, and since your brain is what keeps everything working correctly it doesnt just affect your lungs it affects your entire body.
