y - Y1 = m(x - X1)

y - 6 = 5/2(x - 0) --- POINT SLOPE FORM

y - 6 = 5/2x - 0

+ 6 + 6

y = 5/2x + 6 --- SLOPE INTERCEPT FORM

-5/2x -5/2x

-5/2x + y = 6 --- STANDARD FORM
