bloodborne diseases can be transmitted through cross contamination of reused needles, but it can also be transmitted through a simple mixing of blood. Say person A and person B both have injured hands, and person A has a bloodborne disease, and they shake hands. if person A's blood gets into person B's blood, chances are person B will get the bloodborne disease as well, or they will be exposed to it none the less. it can be spread through contaminated blood donations (however, people are heavily tested and cleared before being allowed to donate blood). Any form of transmission of contaminated blood to non contaminated blood, can and will transmit bloodborne diseases.

I'm also semi sure that they can be spread through mixing other bodily fluids as well.
