If the patient is using any sort of transfer assistive device like a wheelchair or walker make sure to remove the device, or weigh the device and subtract it. Weigh the patient at the same time every day. Remove shoes and excess heavy jewelry or accessories. The nurse may have intake and output be recorded for the patient if weight fluctuates too much.
