Given the equation:

4x+y=0

we have that the slope form of the equation of the line is:

y=mx

In this case, we have the following:

\begin{gathered} 4x+y=0 \\ \Rightarrow y=-4x \end{gathered}

therefore, the equation in slope form is y = -4x
