The standard form is:

ax+by=c

We can transform the equation to the standard form as:

\begin{gathered} y=-(1)/(4)x+3 \\ 4y=-x+3\cdot4 \\ 4y+x=12 \\ x+4y=12 \end{gathered}

Answer: The equation in standard form is x + 4y = 12 (Option C).
