The surface area of a rectangular prism is

S=2lw+2wh+2lh

Where w = 12, l = 4, and h = 4.

\begin{gathered} S=2\cdot4\cdot12+2\cdot12\cdot4+2\cdot4\cdot4 \\ S=96+96+32 \\ S=224in^2 \end{gathered}

Hence, the surface area of the prism is 224 square inches.
