Juan should answer that while the therapy may be 'enough' the patient would (greatly) benefit from more. He should talk to the patient to see if they would be willing to participate in more and then take it up to the administration to see if there is anything that he can do. Hypothetically, to increase funding he should notify (whoever is in charge of department funding) and try to communicate with them. Maybe they should think about seeking out donors.
