Level I Support (Upper Support):

Level I support is provided by the pelvic diaphragm, which includes the levator ani muscles (puborectalis, pubococcygeus, and iliococcygeus).

These muscles form a hammock-like structure that spans the width of the pelvic floor and supports the uterus and upper part of the vagina.

The pubococcygeus muscle, in particular, helps support the anterior (front) part of the vagina and the bladder.

Level II Support (Middle Support):

Level II support is primarily provided by the cardinal-uterosacral ligament complex. These ligaments are located in the pelvis and help hold the uterus and upper vagina in place.

The cardinal ligaments extend from the cervix to the pelvic sidewall, while the uterosacral ligaments attach from the cervix to the sacrum.

These ligaments help anchor the uterus and upper vagina to maintain their positions within the pelvis.

Level III Support (Lower Support):

Level III support includes the perineal body, which is a dense, fibrous structure located between the vagina and the anus.

The perineal body provides support for the lower part of the vagina and the surrounding pelvic structures.

Together, these levels of support help maintain the anatomical position of the vagina and uterus within the pelvis. Weakness or damage to these support structures can lead to pelvic organ prolapse, where one or more pelvic organs, including the uterus or vaginal walls, descend into or protrude from the vaginal canal. Pelvic floor exercises, lifestyle modifications, and in some cases, surgical interventions are used to manage and treat pelvic organ prolapse.
