S=subjective. It cannot be measured because it is a matter of the observer's opinion.

SOAP stands for subjective, objective, assessment, and plan. Health care providers use this format to write out notes in a patient's chart. The part of the SOAP format that's used to record information that can't be measured is subjective. The subjective part represents the subjective observations that are verbally expressed by the patient ( symptoms, their feelings and experiences).
