Based on the foundations of federalism, why is health care under such an ongoing political debate?

Federalism is often associated with big government where its policies cover the entire country. Lately it has been very insistent in implementing its program of universal healthcare. Anti-federalists today want to choose their healthcare plan. Apart from that, they feel that the expense of this program will be placed on the taxpayer and that is what they don’t want.
