I Would say that the most suitable option is 4 one adverb and one adjective clause.

ln fact, it is possible to find 3 clauses within this sentence:
1° It is not right to make a promise (independent clause)
2° unless one is sure he can fulfill the promise. (dependent clause). and 3° he
can fulfill the promise (dependent clause within the other clause)
Let´s take the most prominent clauses: number 1 would be the adjective clause because of the 'right and n° 2 would be the adverbial clause because it is modifying the adjective clause.
