When the nurse palpatates the bladder on an adult client, if she finds a nonpalpable bladder. A nonpalpable bladder is totally normal an a sign of a good and healthy bladder, which means the patient is just find and healthy. If the bladder palpates the nurse should note it as a sign of something being wrong.
