The condition that the client that he or she is experiencing could be hepatic encephalopathy and that the nurse should note other symptoms associated with the elevated serum ammonia level to establish the condition is experiencing such as, consciousness, seizures, decrease protein, hyperreflexia and liver failure or complications.
