To explain the high mortality zone in tropical Africa, we would have to consider the fact that disease might be a measure of population control for primates, including human beings. Whenever a population gets sick and there is no way to get better, a lot of its members might die off, which is ultimately the survival of the fittest. Those who do not succumb to the disease will stay alive whereas the others may not.
