The recommended type of questions, based on American medical association, are indirect questions. If you approach the elderly woman, asking "Are you being abused?" , the woman will most likely say no.

Most appropriate would be: "Are you afraid of anybody at home or anyone who enters your home?" "Has anyone ever physically hurt you?", "Are you alone a lot?"
