Nonfeasance, which simply means failing to perform one's duty. Misfeasance, meaning wrongful performance, in other words screwing up one's duties and not following protocol. Malfeasance, probably the worst failure, it means to intentionally hurt someone by abusing one's powers. These three failures fall under the code of ethics, in other words, doing the right thing. By committing any of these acts, one is breaking that code and doing a poor job.
