The data that should be included on the identification card should be limited to that which is minimally necessary for authentication and identification, such as the officer's initials, the date, and perhaps the time.
The officer's initials will be used to monitor which officials are responsible in handling each aspect of the crime scenes and the date and the time will be used by the investigators to be pinpointed with the statements that are given by the witnesses.
