The nurse should explain to the spouse of the client clearly and with understanding as to why it couldn't be administered. The nurse should tell to the spouse that it is not advisable or contraindicated because administering an anticoagulant to a person with an intracranial hemorrhage will only cause the bleeding to increase and would not help the situation.
