Two circles with a radius equal to the radius of the of the cylinder and a rectangle with a height equal to the height of the cylinder and a width equal to the circumference of the cylinder.

So it is the sum of the area of two circles and a rectangle.

The surface area of a cylinder includes the bases which are circles and the side which is a rectangle
