Disclosure is telling the information that is important for the insurance company to know or it's  answers to the important question they ask, for example, "are you disabled?" or "Have you ever been to federal prison?" It is important to disclose this information because the insurance you get is based on the information you provide, providing false information, or even none at all can void your insurance policy.
