The statement which is true is C. Themes may be specific to one culture or apply to multiple cultures.
This is the correct answer because in A, themes can be obvious sometimes. In B, the theme doesn't have to be referred to in the middle of a work, it can be stated anywhere. In D, the theme doesn't have to be stated directly.
