If by new directory you mean new folder then (A.
Click on the New Folder icon in the File menu.) is the right way to add a new folder. From there you can right click the new folder and rename it what ever you want.
For the second question there are two
procedures that are involved in saving a file for the first time
1. giving the file a name
2. navigating to the place where you want the file to be saved
