Answer: The appropriate position the nurse should do is the side-lying position and the semi-Fowler position helps to maintain the head in functional body alignment and facilitates respiration; also gravity moves the abdominal organs down and away from the diaphragm, facilitating respiratory excursion. Oral intake should not be attempted until the return of the gag reflex because the client could aspirate. Although some slight irritation may occur following this test, there is usually no painful squeal; oral intake would not be withheld because of painful swallowing, although the consistency of food may be changed. The statements that the performance of the graduate nurse is inappropriate because the client may be anxious or thirsty are not correct; additional factors must be considered.
