Under this method, a physician charges directly for treatment services rendered to the patient and the patient is legally responsible for the payment. While the insurance company reimburse the patient for medical expenses incurred. This is a fee for service system where two contracts is made, one for the contract between the physician and the patient, and the other is between the patient and the insurance company.
