The answer is: sensation and perception blend into one continuous process.
The perception on a certain stimulus that human felt is the extension of the sensation that transferred through our nervous system.
For example, when we feels something cold in our skin without seeing the object that cause it, we would automatically would perceive it as something that we recognize to be low in temperature, such as ice.
