y = 1/2x + b is the general form of the equation you want. The slope is the number in front of the x.
when x = 3, y = 4 will get you b.
4 = 1/2(3) + b
4 = 3/2 + b
b = 4 - 3/2 = 2.5
y = 1/2(x) + 2.5
