genuineness is a personality trait that make people sincerely had the intent to help others and present themselves exactly as their own true nature (without hiding their personalities).

In the case above, seeing therapist before seeing patients could be interpretation as Burns' effort to let go of his own emotional burden before seeing the patients so he does not have to pretend that he currently cares with what his patients are saying during the session.
