The Hills Like White Elephants uses metaphors and allusions to talk about an unplanned pregnancy and it can be inferred that the woman is supposed to have an abortion though it's not completely clear whether or not she has one in the end. This pregnancy, and more specifically the impending abortion, causes tension in the relationship.
