The standard form is y = x 2 4 − 3 x + 10 y= 4 x 2 −3x+10.

The general form is x 2 − 12 x − 4 y + 40 = 0 x 2 −12x−4y+40=0.

The vertex form is y = ( x − 6 ) 2 4 + 1 y= 4 (x−6) 2 +1.
